We are excited to share that the Early Learning Hub and Pollywog have joined Connect Oregon. As proud partners, we can better serve the people in our community by collaborating with providers across health and social services. Partners are connected through a shared technology platform to coordinate care and address people’s social needs. Through this Unite Us-powered network, we will refer and connect our clients to the services they need, track outcomes of all electronic referrals and services delivered, and measure our community’s impact.

The network contains partners who provide a broad range of services such as housing, employment, food assistance, behavioral health, utilities, and more. The network also creates a community-wide data set to better understand the needs of community members and gaps in service availability to address those needs.

This is going to benefit our community’s health for years to come. We can’t wait to work together with this new tool.
